We are thrilled to have your child join the PLAY community! Our programs provide a safe, inclusive space for growth and connection. To ensure a safe and successful mentorship experience, please complete the two-part registration process outlined below.

Requirement: You will need to sign and return the final page of this document in Step 3.

Please complete our secure online survey. This helps us match your child with the right mentor based on their interests and safety needs.
What’s Inside: 9 sections covering contact details, hobbies, goals, and essential medical/allergy information.
Legal Consent: The final section of the form includes mandatory checkboxes for Program Consent, Media Release, and Privacy.
Step 3: Submit Signed Agreement Page

To finalize your registration, we require a signed copy of the Handbook Agreement.
Print or Sign Digitally: Complete the final page of the Handbook from Step 1.
Rename Your File: Please use the format: StudentLastname_StudentFirstname_Handbook.
Submit via Email: Attach the signed page and send it to info@playouth.org.
Subject Line: Use: [Student Name] - Signed Handbook Agreement.

Once your submission is received, our team will:
Review the medical and emergency information for completeness.
Send a confirmation email to the parent/guardian within 3 to 5 business days.
Provide a "Welcome Package" containing program dates, location details, and staff contact information.
